Care Provision

In all of our supported housing schemes Care Plans are drafted within 30 working days of admission to the scheme. This is done with the involvement of the resident, their relative, next of kin, key worker and external agencies.
  
These Care Plans are reviewed and updated on an ongoing basis to ensure the changing needs of residents are addressed. 

The Association strives to hold annual multi-disciplinary meetings to review Care Plans for each resident. We also try to hold resident/family meetings at least 6 monthly to encourage participation by residents and we regularly undertake resident satisfaction surveys.
  
Through our Tenant Participation Policy we encourage residents and/or their families/advocates to participate in the Association's decision making processes.
